Pre-Operative Assessment Health Triage Survey

We are launching our first health survey on the My RFL Care patient portal in June.

The Pre-Operative Assessment (POA) Health Triage Survey will be sent to eligible patients who are advised they need to have an operation or procedure* by their Royal Free London clinician.  The survey responses will help the POA team to book patients into the right type of appointment the first time round based on their current health status and needs.

What to expect

If you are eligible to receive the survey, you will receive a text message with a link included inviting you to complete it. The survey should only take between 5-10 minutes to complete. Please provide as accurate and full responses as you can. The information you provide us will help your healthcare team to correctly assess your needs. The survey link will expire at 23:59 the day after you received the text message.

  • If you are already registered on the portal, click the hyperlink included in the text message and this will take you straight through the 'pre-operative assessment health triage survey'.
  • If you are not yet registered on the portal, you will be asked to sign up first, before being taken through to the survey. Please click the link included in the text message to register. Once registered, you will be redirected to the survey, or you can access this by clicking 'My Questionnaires' from the drop-down menu on the My RFL Care patient portal.

If you aren't able to complete the survey before it expires, please do not worry. You will still receive an appointment for your pre-operative assessment.

About the Pre-Operative Assessment Health Triage Survey

The Pre-Operative Assessment (POA) Health Triage Survey' has been designed by our expert team of clinicians and patient feedback. You will be asked a series of questions about your current health conditions and some social questions. Please answer the questions as fully and honestly as possible so we can book you into the correct type of pre-assessment appointment.

If you have any feedback on the survey or your experience of completing it, please contact: Please note we cannot provide medical assistance or answer any questions about your condition, this inbox is monitored for feedback only.

What happens if I don't complete the survey?

Please do not worry if you do not complete the online survey. You will still receive an appointment for your pre-operative assessment. This will most likely be a telephone appointment in the first instance, but you may also be required to attend the hospital if you need to be seen face to face.

*Eligibility for the survey

The Pre-Operative Assessment (POA) Health Triage Survey will only be sent to those patients who are suitable to receive it, for example patients who may not require a general anaesthetic or who require urgent treatment will not receive it.
