To make a change or cancel an appointment for a person that is unable to do this for them self, please contact the out-patient appointment centre by phone.
Please note that for security purposes you will need to be able to:
- Provide the demographics of the patient you are assisting.
- Confirm that you have authority to change the appointment for the patient.
All calls are recorded for security purposes. If the patient is able to complete the change of appointment form with assistance, they can do it here.
