Referral details
Staff wanting to access the services of the occupational health and wellbeing centre will find that referral is quick and easy. All referrals to the centre are strictly confidential.
Barnet, Chase Farm and Edgware hospitals
If you need to speak to an occupational health advisor or book an appointment at Barnet Hospital, Chase Farm Hospital or Edgware Community Hospital, please contact the occupational health and wellbeing centre on 0208 375 1821/2468.
Royal Free Hospital
If you need to speak to an occupational health advisor or book an appointment at the Royal Free Hospital, please contact the occupational health and wellbeing centre on 020 7794 0500 and ask for extension 33148.
Management referral
Our referral forms for managers have been designed for easy use. The more relevant information we have the better we can respond to your referral and tailor our service to your need. If you would like help with completing the form, or to discuss whether a referral is appropriate, please ring and speak to any member of the team at the occupational health and wellbeing centre.
All management referrals need to be discussed and agreed by the line manager of the member of staff you are referring. The line manager must sign the referral form and keep a copy for their records.
